Dr. Elijah Wilder, D.O., was born and raised in Raleigh, North Carolina. He earned a Bachelor of Science degree in Biology from Campbell University. He attended medical school at the Philadelphia College of Osteopathic Medicine. He completed Psychiatry residency training at prestigious Duke University, followed by Addiction Psychiatry fellowship subspecialty training at the University of Cincinnati. Dr. Wilder is dual board-certified in both Psychiatry and Addiction Psychiatry by the American Board of Psychiatry and Neurology, and he is also a member of the American Psychiatric Association and the American Medical Association. He has over 10 years of experience providing specialized care in mental health and addiction treatment. Prior to joining Renal Psychiatry Services, he has worked in various care settings including inpatient, outpatient, long-term care, detoxification, and residential facilities, and he has recently served as Regional Medical Director for a large addiction treatment consortium with countless facilities across the entire country. He also holds an ongoing appointment as Assistant Clinical Professor at the University of Cincinnati - Department of Psychiatry and Behavioral Neuroscience.
Dr. Wilder comes to Renal Psychiatry Services with a strong passion, dedication, and commitment to providing compassionate, high quality, individualized, and evidenced based patient care. He also prides himself on pioneering and delivering the most up-to-date and cutting-edge treatments to help patients live beyond their mental health and substance abuse issues as successfully as possible.

Kristin OBrien is the Chief Operating Officer for ActivateCR. She is an Air Force brat that lived in multiple states growing up. She earned her Bachelor of Science degree from St. Francis in Peoria, IL. She has worked in the medical field for over 25 years in multiple positions. She has worked in admissions, clinically, and administratively throughout her career. She has managed vendors and negotiations; collaborated with professionals, contractors, consultants, and fellow co-workers, striving to create the most efficient processes possible. Working in the Emergency Room and Cardiac Catheterization Lab are high stress positions requiring attention to detail, professionalism, and a desire to assist and provide compassionate care to the patients. Currently, she is working as Chief Operating Officer of a transitional healthcare corporation, and now additionally ActivateCR.

Healthcare Operations Leader
& Dialysis ExpertWith over two decades of dedicated service in the dialysis sector, Paul Gordon brings a unique blend of clinical expertise and operational acumen to the healthcare industry. His career spans impactful leadership roles, including Director of Clinical Projects and Patient Services Director, where he has consistently driven initiatives that enhance operational efficiency, regulatory compliance, and patient care.
At Dialyze Direct, Paul led transformative efforts across 175 facilities, standardizing processes, transitioning admissions documentation to electronic systems, and guiding the organization through the complexities of COVID-19 regulatory adaptations. At Fresenius Kidney Care, he managed the psychosocial needs of diverse patient populations, spearheading cross-functional initiatives to improve patient services and outcomes. During his tenure at Quality Insights Renal Network 4, Paul secured and exceeded CMS-ESRD contract objectives, provided expert guidance on CMS Conditions for Coverage, and successfully led the Patient Services Department.
Paul’s career is defined by his commitment to excellence, his passion for patient care, and his ability to translate strategic objectives into actionable plans that drive meaningful impact across the healthcare continuum.

Jennifer is a dual-certified nurse practitioner with acute care and psychiatric-mental health nurse practitioner certifications. She graduated from Texas Christian University. She received her master’s in nursing from UT Arlington. She also completed her training for the acute care and psychiatric mental health certifications through UT Arlington. She has a passion for helping others and empathy for those with mental health needs

With over a decade of experience as a Licensed Clinical Social Worker in nephrology, she specializes in supporting CKD and ESKD patients through integrated, patient-centered care. She earned a Doctorate in Educational Leadership, which enhances her ability to participate with interdisciplinary teams (IDT) with a focus on servant leadership.
This year, she was awarded the Nephrology Social Worker - Certification (NSW-C) by the National Kidney Foundation. Her work bridges clinical expertise and compassionate advocacy, ensuring quality outcomes and holistic support for individuals navigating chronic kidney disease.
She resides in Mission, Texas. Outside of work, she enjoys spending time with her family, exploring the beach, and traveling to recharge and stay inspired.

Rehan Shah, MD, received his training at SUNY Downstate Medical Center. He has gained valuable experience by treating a very diverse population with vast pathology in the New York area. He currently serves as Medical Director on inpatient dialysis units, giving him firsthand knowledge and expertise in kidney disease and dialysis. He is currently the Founder and CEO of Renviva, a physician-owned and physician-led dialysis provider focused on bringing dialysis services to the home and onsite at SNFs.
Most recently, CardioRenal Therapeutics, LLC was formed to bring not only dialysis services to SNFs but also to provide onsite cardiology and nephrology care. This full spectrum of care provides an integrated care approach to SNFs, thereby improving patient outcomes and decreasing costs by reducing hospital admissions.
